Training Evaluation - Forms and Questionnaires
These resources are sample evaluation forms and guides to adapt for your own use. Course summary evaluations, focus group questions, and expert observation tools are included. There is a trainer’s competency checklist and trainer attributes competency self-assessment. These forms can encourage trainers to strengthen their training and communication skills and strive for improvement.
- Daily Evaluation Form File Type: Word Document Pages: 1 A sample form for collecting trainee feedback at the end of the day. Use this quick tool to obtain feedback from each participant about their training experience re: enjoyment, confusion, value, applicable knowledge and skills. This tool is particularly useful for each day of a multiple-day training.
- Training Evaluation Form: Skills, Attitude, Comfort File Type: PDF Document* Pages: 1 A self-assessment rating form for participants to indicate their pre-test and post-test comfort, knowledge, and skills, e.g., counseling, managing, and providing services to clients as well as knowledge about the specific HIV/AIDS topic.
- Training Evaluation and Learning Self Assessment File Type: Word Document Pages: 2 A sample form for evaluating a training session, produced by the Northwest AIDS Education and Training Center. This tool asks participants to evaluate the trainer’s expertise, how the participant increased his or her knowledge and skills, anticipated practice changes, strengths and weaknesses of the presentation, and ideas for course improvements.
- Post-Training Summary Evaluation File Type: Word Document Pages: 1 A very simple three-question evaluation used at the end of a training course. Covers important things learned, amount of materials used, and anticipated impact on participant’s job. This tool can also be used as a daily evaluation form with or without the "Daily Evaluation Form."
- Train-the-Trainer (TOT) Summary Evaluation #1: Short File Type: Word Document Pages: 2 A simple evaluation form for use at the end of a train-the-trainer workshop. In general, it covers most valuable learning, strategies to deal with workplace HIV+ stigma, usefulness of course, applicability, and suggested changes/improvements to the course. Use the Train-the-Trainer (TOT) Summary Evaluation #2: Long form for a more extensive TOT evaluation form.
- Train-the-Trainer (TOT) Summary Evaluation #2: Long File Type: Word Document Pages: 2 A sample evaluation form for use at the end of an HIV train-the-trainer workshop for nurses. Longer and more structured than Train-the-Trainer (TOT) Summary Evaluation #1 , this form provides rating grids about course content, materials, presentation, self-assessment regarding participant’s knowledge and skills, etc., and questions regarding HIV care and implications for nursing. This form can easily be adapted to trainings for other healthcare professionals.
- Training Observation Instrument File Type: Word Document Pages: 5 A form used to observe and analyse a training exercise. The observer might be a supervisor, trainer, regional staff member, etc. The form provides note-taking instructions and spaces to identify the learning objective, impressions, questions, and a summary analysis based on observation notes. This tool can be the basis of very valuable feedback for the trainer.
- Expert Observer Rating Tool File Type: Word Document Pages: 15 A comprehensive observation and rating tool for evaluating a training session. Evaluation categories address training components and issues such as session content; what strategies, materials, and activities were used; trainer’s approach; pace of training; and how well participants learned.
- Group Activity Observation Form File Type: Word Document Pages: 2 A form used to guide an observation of a group activity. Structured a bit differently than the Training Exercise Observation Instrument, this form provides a template for identifying the learning objectives of the exercise, how participants interacted, how the trainer handled any confusion, the collaborative work of the group, time allotted, and the outcome of the activity.
- Evaluation using Focus Groups: Topic Guide File Type: Word Document Pages: 1 A list of questions to help you guide a focus group through a training evaluation. Topics include eight basic areas, e.g., relevance, strengths, weaknesses, and possible implementation.
- Trainer's Skills: Competencies Checklist File Type: Word Document Pages: 2 A checklist evaluators can use when observing and assessing a trainer’s presentation skills. Specific areas include delivery, use of visual aids, body language, and audience participation. There is also a space for narrative comments on how the trainer might improve his or her delivery. This is intended as a useful tool for giving supportive and constructive feedback.
- Trainer Attributes: Competencies Self-Assessment File Type: Word Document Pages: 4 Self–assessment tool for trainers to examine their strengths and how to improve their strengths in a variety of areas. Trainers are asked about their preparedness, knowledge of their audience, nonjudgmental approach, self-awareness regarding bias, group skills, etc. Use this tool as a post-training evaluation or as a guide when considering candidates to present your training.
- Instructional Design & Materials Evaluation Form File Type: Word Document Pages: 3 A form for evaluating instructional design elements, content, and the evaluation methodology. This form is to be used with Instructional Design and Materials Evaluation Guidelines.
- Instructional Design & Materials Evaluation Guidelines File Type: Word Document Pages: 9 Guidelines for evaluating instructional design elements, content, and the evaluation methodology. These guidelines are to be used with Instructional Design and Materials Evaluation Form.
- Trainer Debriefing Guide File Type: Word Document Pages: 15 A brief questionnaire to guide trainers through a post-class or post-session debriefing. This tool will help trainers take a look at how the training session or workshop went. There are six basic categories, e.g., what did and did not work, understanding of clinical components, and group/individual participation.
- Training Evaluation for Healthcare Professionals File Type: Word Document Pages: 3 A training evaluation form for healthcare professionals. Participants can use this form to provide comments on the strengths of a training, possible improvements, usefulness of a training, knowledge acquired, and how new information or skills will be used in the clinician’s workplace.
